In PICO ENERGY, employees and contractors report both incidents and near misses. Although a near miss is an event without immediate consequences, we recognize it could have resulted in an injury, property damage or environmental impact. Furthermore, if a potential hazard is identified, we believe reporting is not enough. We will implement corrective actions to address the root cause and reduce the risk of recurrence.
LFI process
Learning from incidents, or LFI, is just one of many activities in managing safety in PICO ENERGY. LFI is defined as “a process through which employees and the organization as a whole seek to understand any negative safety events that have taken place to prevent similar future events.” The outcome usually should lead to changes in behavior or technical process in any organization if all stages of LFI are properly implemented.
